05/06/2014 - There are currently THREE data sharing options available - all can be opted in or out of.
1. Summary Care Record – Have you made your choice?
Summary Care Record (SCR) is an electronic record of important information about a patients health. It will initially have information about current medications, allergies and any bad reactions to medicines. Additional information may be added over time if a patient gives their consent.
George Clare Surgery is planning to upload Summary Care Records from 18/06/2012.

2. Enhanced Data Sharing Model (eDSM)
Your health record includes your medical history, details about your medication and any allergies you may have. You can now choose whether to share these full medical details.
We use a secure electronic health records system called SystmOne. With your permission, this system can allow clinicians to share your full record held here with other healthcare services who are providing care for you. These other services will ask your permission to view your record.
Many organisations may use SystmOne including some GP practices, Out of hours services, Children's services, Community services and some hospitals. Sharing your health record will help us deliver the best level of care for you.
You have two choices which allow you to control how your record is shared. You can change these choices at any time by letting the relevant practice or service know.
Sharing OUT - this controls whether your information recorded at this practice or service can shared with other healthcare services.
Sharing IN - this determines whether or not this practice or service can view information in your record that has been entered by other services who are providing care for you, or who may provide care for you in the future.

3. Better Information means Better Care
Information about you helps us to provide better care.
Confidential information from your medical records can be used by the NHS to improve the services offered so we can provide the best possible care for everyone.
This information along with your postcode and NHS number but not your name, are sent to a secure system where it can be linked with other health information.
This allows those planning NHS services or carrying out medical research to use information from different parts of the NHS in a way which does not identify you.
You have a choice. If you are happy for your information to be used in this way you do not have to do anything.

Patient Access to the Summary Care Record
Patients will be able to access their personal Summary Care Records as of 30 September 2014.
In order to access this part of your record you will need to request access via Systmonline and permission for this will be granted at the clinician s discretion. The record contains information about any medicines you are taking, allergies you suffer from and any bad reactions to medicines you have had.
Please note - to view your summary record you will need to agree to share your Summary Care Record – please contact the practice should you wish to be able to access your Summary Care Record.
