Registering With George Clare Surgery
We are committed to providing the highest standard of medical care for our patients. The practice catchment area covers the town of Chatteris and the surrounding villages of Benwick, Doddington, Manea, Mepal, Ramsey (defined area) Ramsey Forty Foot, Somersham, Stonea, Sutton, Warboys (defined area), Wardy Hill, Wimblington and Witcham.
The practice accepts some new patients from Ramsey and Warboys - please check that we will cover your new address before registering.
Patients wishing to register with a GP and who live in our practice area will be asked to complete a registration form and a new patient questionnaire for each family member with details of full name, date of birth, new address, previous home address and previous doctors name and address. When completing the registration form please include your NHS number (this can be found on your medical card - if you have one).
Where possible Proof of ID is required for all patients registering aged 16 and over. Acceptable forms of ID are Photo driving licence, Passport, Visa, European personal identity card, bank statement or utility bill. We have a household registration policy where we endeavour to register all household members with the same doctor e.g. students returning home from university etc.
If you are not normally resident in the UK or have not been registered with an NHS doctor before or do not have either your NHS card or a NHS number we may require some proof of identity and your eligibility to receive NHS care.
All patients aged between 40 - 74 years will be offered a free NHS health check.
We operate an open access policy whereby any patient can request to see a doctor of their choice. Each patient is registered with a named doctor and, where possible, a patient should see that doctor in order to provide continuity of care. If for any reason a doctor is absent, patients will be seen by another doctor.
From 1st April 2015 All practices are required to allocate a named, accountable GP to all patients (including children). The named GP will be responsible for the overall care of the patient.
Patients will still be able to arrange appointments with a doctor of their choosing.
If you are ill while away from home and need to see a doctor you are able to register as a temporary patient.
You can be registered as a temporary patient for either less than 15 days or for up to three months. Temporary registration will allow you to be on the temporary practice list and still remain a patient of your permanent GP. We will need you to complete a Temporary Registration form,
Moving Out of our Practice Area
For patients who move from our Practice Area, it is considered more clinically appropriate and practical for your healthcare needs if you register with a GP practice nearer to your home that can provide home visits or urgent appointments more convenient for you.
We therefore advise you to register with a practice in your new locality as soon as you are able.
